Dear Sir,
I am happy to inform you that you have received the unanimous approval of the Examining Board. Your commission as Surgeons Mate will be issued from the office of the Adagt. Genl. whenever you are affronted by the Col. of a Regiment, which is completed and accepted into the service of the State.
